Outlast Trials Relapse is the prestige system added on 22 April 2025 with Season 3, Project Relapse. At the maximum Therapy Level 99 you can choose to start over, in return for a prestige outfit, a Relapse plaque and a permanent token.
What Outlast Trials Relapse resets
A Relapse resets your Therapy Level to 1 and resets your upgrades: amps, rigs, prescriptions and vouchers. Currencies other than vouchers, your stats and your Escalation save are kept. A permanent token makes one upgrade survive the reset.

Is it optional?
Yes. You can decline, and Escalation, the Trial Maker and Invasion stay unlocked. If you start over, plan which amp or rig a token should protect with the loadout builder.
Relapse and Rebirth
Therapy Levels 2 to 15 pay 3 vouchers each, so the first levels after a reset come quickly; the vouchers guide has the full sum. Rebirth is a different system: it needs 100 Rebirth Tokens, can be solo or communal (added with Project Diarchy on 25 June 2024) and pays outfits, 100 Stamps and 10,000 XP, among other rewards.
